Arranging an Assessment

Once your Funding Authority has confirmed that you need to arrange a Needs Assessment and you would like to be assessed at the Staffordshire Regional Access Centre, you should contact us.  We will endeavour to match you with the most suitable Assessor available for your disability.  The cost of your needs assessment will be paid for by your Funding Authority so you must wait for a letter confirming your eligibility before arranging for an assessment at the Centre.

We will offer you an appointment and ask you to send us copies of the following information and forms prior to your assessment:

• A copy of your Disabled Students' Allowances eligibility letter from your Funding Authority (Local Authority, NHS or Student Finance England).

• You will need to provide a copy of the evidence for each impairment or specific learning difficulty that you provided to your Funding Authority to prove to them that you are eligible for the Disabled Students’ Allowances. 

• A completed pre assessment questionnaire (DOC, file size: 740KB)

• A copy of your previous needs assessment report (if applicable).

You can return this information to us by email, in person or by post. 

We cannot assess your needs without this information and we may have to reschedule your appointment if you do not return this information to us promptly.  We ask that you return this information to us within 5 working days of booking your appointment.

  
We can offer you an appointment within 5 working days. However, please note that not all of our Assessors are trained to assess all disabilities; the appointment waiting times vary, depending on disability, assessment location, Assessor availability and cancellations.
